Behavioral Health Home Services (BHHO)

The Behavioral Health Home Program (BHHO) utilizes a team approach to the provision of case management and serves both adults and children. Clients in the BHHO program receive case management from the Health Home Coordinator, and also may engage supplementary services such as a Nurse Care Manager and Certified Intentional Peer Support Specialists. The BHHO is designed for clients whose needs may be less emergent than those in the Section 17 or Section 13 programs, but who would continue to benefit from support as they work towards increased independence. In this program, clients will see their case manager at least once per month, but dependent on need, can access the other team members. 

 

The Nurse Care Manager can help clients prepare for important medical appointments, understand physician’s orders, and check in on clients with chronic health conditions. The Peer Support Specialists are individuals who have previously engaged with the mental health system as a consumer themselves, or as a parent of a child who has special needs. They are trained to listen to you and support you from the perspective of someone who’s had to do it all before, and they can bring special insight to some issues and processes based on their own personal experiences. 

 

To qualify for the BHHO program, a client must first be assessed by a clinician to determine whether they have a mental health condition(s) that would be accepted for this service under the guidelines of Section 92.

Parental Education and Support

Our Family Peer Support Specialist has parented children with special needs and has gone through the special education process as a parent herself. She has navigated this system and can be a wonderful resource to parents who may be struggling to understand the complex options available, including IEPs, 504 plans, and the different educational opportunities available to children with special needs. Even if a parent is simply struggling to communicate with their children’s school(s), the Family Peer Support Specialist can help families connect with schools to make sense of their options and parental rights in the educational system

Community Integration and Development of Natural Supports

Our Certified Individual Peer Support Specialist is here to help clients enrich their lives. As a person with a mental health condition who has successfully learned how to cope with their symptoms, the CIPSS has a wealth of knowledge around engaging with systems like DHHS and agency mental health organizations. She is a safe person to talk to who is not a case manager or a counselor, but a peer who can speak to you on your terms and at your level. The CIPSS often takes clients into the community to build relationships with others, or just to help clients get out of the house for a while to experience something different. If you would like to talk to someone on more of a friendly level while exploring your community, the CIPSS is eager to support you!

Medical Support and Understanding

Our Nurse Care Manager is an RN (Registered Nurse) who brings her specialized medical knowledge to the treatment team. The Nurse Care Manager can help clients understand and prepare for upcoming appointments or procedures, managing after-care instructions, or in dealing with chronic medical conditions like diabetes or ongoing pain. Especially for older patients struggling with keeping track of a variety of prescriptions, having a regular check-in with the Nurse Care Manager can help clients keep their medical needs organized and understandable.

Consultation Services

The BHHO team has consultants at their disposal when needs come up that require more specialized attention. The team can draw on the expertise of a psychiatrist, a physician, and a licensed clinical social worker to generate ideas and solutions for issues the clients may experience that are out of the ordinary and needing more thoughtful approaches.
